Completing the Well and Septic System Requires a Useful Final Report

Next to the actual Home Inspection Report, the Well and Septic Report is the most valuable tool that the inspector delivers to the homeowner. It starts with the homeowner/occupant questionnaire and isn’t complete until the Site and System and Final Report is complete. Old Line Home Inspections is certified by the Maryland Onsite Wastewater Professionals Association (MOWPA) to do proper well and septic inspections for property transfers in the State of Maryland. We follow the procedures that have been put in place by the Maryland Department of the Environment utilizing the following checklist and final report requirements.

Site and System Evaluation/Inspection Checklist

The inspector will evaluate the system using a detailed checklist that includes the following:

  1. The septic tank will be inspected to ensure that it is structurally sound and operational with required inlet and outlet baffles/tees and or necessary components.
  2. Tanks and access riser connections should be watertight and not leak.
  3. Liquid levels in the tanks are measured and should reflect that the system is functioning according to design.
  4. Toilets or drains on the lowest level of the house must flush or drain adequately.
  5. The drainage field is inspected, and the report will contain its findings.
  6. If the house has been vacant for a long time or if the well is in questionable condition a hydraulic load test will be performed.


The final report for an OSDS inspection will include the following information:

  1. The address of the property and date of the site inspection
  2. The type, size, structural integrity and number of system components
  3. Information from current/recent occupants of the house/facility about the system operation & usage (IMPORTANT!)
  4. Information obtained from the local health department (or permitting agency) concerning the OSDS. It should be noted if no information was available or if the information was requested but not received as of the date of the final report.
  5. A sketch of the OSDS layout showing the location of all system components relative to the house and/or other prominent site features (eg., sheds, pools, etc)
  6. A conclusion/comments section that specifies what was observed. At times, it will be necessary to characterize the condition of the system. Terminology will include: system is acceptable, system is acceptable with concerns, further evaluation is needed, or system condition or performance is unacceptable.
